• A statement that the information in the notification is accurate, and under penalty of perjury, that you are authorized to act on behalf of the owner.

2. Counter-Notification

If content was removed by mistake or misidentification, a counter-notice generally identifies the material, includes contact details, the required jurisdiction and mistake statements, and a signature. The worksheet does not transmit or file it; deliver a completed notice independently to the appropriate recipient.

3. Repeat Infringer Policy
